Hyperline Policy States,

This form constitutes an official authorization for Hyperline to charge your credit card for the payment of services requested. It is the only acceptable form for credit card payment.

This form must be fully completed, signed and sent via Fax or E-mail to Hyperline for review. Once The Credit Card Acceptance Form  is approved you then complete and return the Credit Application & Agreement Form for final review.

For more information on programs, procedures and methods of payment accepted please talk to a Hyperline service representative.

The form must contain the Canadian or US dollar amount authorized for services payment.
